Uniform Guidelines ~ expectations for daily and Mass day attire
Medication Authorization Form ~ necessary form filled out by a doctor for school staff to administer any medicine to a child.
Permission to Pick Up Form ~ necessary for our school staff to release your child to someone else's care. May be faxed to 639-1242.

Meal Magic for Hot Lunch & Milk ~ Create an online account to check your child's remaining balance, deposit additional funds, and view account activity. Online payments incur a processing fee. Although the hot lunch program is administered by the public school, payments may still be dropped off at the St. Basil office. Simply make your check out to SHPS and note your child's name and grade on your check. Each hot lunch is $2, each milk is $.40. Click here to view the hot lunch menu.
